Poynette Middle School Athletics

Student/Athletes will need to turn in the following items:

1. Green Physical card (signed by doctor) or a beige alternate year card (signed by parents)
THIS MUST BE DONE PRIOR TO ANY PARTICIPATION
The purpose of this card is to let the school know that a student has been determined healthy enough to participate. This determination needs to be made by a Doctor. So students participating in their first sport or for the first time for the school needs to turn in a green card. This needs to happen every other year. On the off year, an alternate card is all that is required which is signed by the parents.

2. Athletic/Activity Code
NEEDS TO BE TURNED IN BY THE FIRST GAME OF THE YEAR
This will be handed out and explained at the parents’ meeting early in the season. This is board policy and students need to follow it year round and fall under the guidelines of the policy even if they do not sign it.

3. Emergency/Medical History Form
NEEDS TO BE TURNED IN BY THE FIRST GAME OF THE YEAR

4. Athletic Fee
NEEDS TO BE TURNED IN BY THE FIRST GAME OF THE YEAR
Another board policy, that states each middle school student will need to pay a fee for each sport they participate in.
