General Registration Information

Welcome to our district. Parents with school-age children who are new to the community, and those parents moving within the school district, are encouraged to register at their children’s new school as soon as possible. Parents with questions about school boundaries may call the our district office at 608-635-4347 x202 for assistance. If a student has special education needs, additional information is available by contacting Student Services 608-635-4347x201 .
  
A certified birth certificate, baptismal certificate, passport, or immigration certificate, as well as medical record is required by law.  A recent report card from the last school attended is helpful. A child must be five years old on or before September 1, for entrance into kindergarten, or six years old on or before September 1 for entrance into first grade. 
 
Since good physical health is vital for a successful school career, it is strongly recommended that children have a physical examination prior to entering kindergarten, third, sixth, and ninth grades.
Wisconsin Statute requires that all students in grades K-12 be immunized against:
 
·         Diptheria
·         Pertussis        
·         Tetanus
·         Polio
·         Varicella (chicken pox)
·         Mumps
·         Measles
·         Rubella
·         Hepatitis B 
At registration time, parents will be given a form, which will request information on the dates (day, month, and year) when the immunizations were administered.
